Dear Parent/Guardian

 

The Human Papilloma Virus (HPV) programme requires two doses for 12-15 year olds. If your child is 15+ or immunosuppressed they will require 3 doses regardless of their age when they receive the first dose.  

 

The School Age Immunisation Team will be visiting your child’s school on Wednesday 29th June 2022  to administer Dose 2 of the HPV vaccination programme. The clinic that was originally planned for Wednesday 4th May 2022 will not go ahead and will instead take place on the new date.

 

You would have completed a consent form for your child to receive Dose 1 of the programme in year 8, we now need you to consent for dose 2. Without this consent we will be unable to vaccinate your young person with the 2nd part of the HPV vaccine to complete the course. 

 

If your child has not yet received any HPV vaccinations and you would like them to start the programme now, please complete a consent form using the link below, you will be consenting for the full HPV programme.
