Dear Parent/Guardian
Your child is due their vaccination to protect against Human Papilloma Virus (HPV). This is offered in year 8 as part of the national immunisation programme and is free on the NHS.
This vaccine requires two doses; the second dose is given 6 – 24 months after the first. If your child is immunosuppressed they will require 3 doses.

You must have parental responsibility or delegated authority for immunisations to consent for the vaccinations.

The School Age Immunisation Team has arranged with school to visit on Thursday 16th June 2022, to administer this vaccination.
Electronic Consent - The School Age Immunisation Team now operates paperless vaccination programmes and the use of e-Consent is quick and easy to use, and can be completed on any electronic device.

Please complete the e-consent form by 09:00am on Monday 13th June, consent forms will not be accepted via this link after this date/time
Please ensure you know your child NHS number prior to booking, you can find this in your child’s “Red Book” or by using this link: https://www.nhs.uk/nhs-services/online-services/find-nhs-number/
Please ensure only one parent / guardian books their child’s vaccination appointment to reduce the risk of duplicate appointments / vaccinations.
